Kaiser Permanente Community Fund at the Northwest Health Foundation
The mission of the Kaiser Permanente Community Fund (KPCF) at the Northwest Health Foundation is to improve the health of communities throughout the region, with a specific focus on addressing the “upstream” factors that create or inhibit community health, also known as the social determinants of health. Since the first grant cycle 10 years ago, the fund has provided over $32 million to 150 organizations, granting approximately $2.5 million per year.
Over the past eight years, the Kaiser Permanente Community Fund has advanced important initiatives in many social determinants of health. In 2013, the KPCF Advisors selected three specific social determinants of health that build on community momentum and have the greatest opportunity for long-term impact. They are Early Life, Educational Attainment, and Economic Opportunity.
