HONOLULU — Kaiser Permanente’s Moanalua Medical Center has received the 2018 Get With The Guidelines® – Heart Failure Gold Plus Quality Achievement Award from the American Heart Association. This marks the seventh consecutive year Moanalua Medical Center has been honored with a Gold Plus distinction, which recognizes the organization’s success in implementing a high standard of care for treatment of heart failure.
The Get With The Guidelines program is designed to help hospital teams meet specific quality achievement measures for the diagnosis and treatment of heart failure patients according to nationally recognized, research-based guidelines. The measures include proper use of medications, risk reduction therapies, patient education, follow-up care and other care transition interventions, with the goal of speeding recovery and reducing hospital readmissions. Hospitals receiving the Gold Plus recognition must meet two standards — treating heart failure patients with 85 percent or higher compliance to core standard levels of care for 24 consecutive months; and 75 percent compliance to four out of nine heart failure measures during a 12-month period.
