How to Apply for a Kaiser Permanente Grant

The majority of Kaiser Permanente Northern California regional grantmaking is initiated by Community Benefit Programs with an invited application from a member of our staff. Grant invitations are generally related to a pressing community need or for specific services or programs.

We are also able to respond to a limited number of requests from nonprofit organizations in Kaiser Permanente’s Northern California service area, particularly those serving more than one local service area and having implications for replication and/or dissemination across the Northern California region.

Process to Apply

Once you have determined that your project is eligible for a Northern California Community Benefit Programs Regional grant, we use two different processes for applications, depending on the source of the request.

  1. Letter of Intent (LOI). If your organization meets our basic eligibility requirements and is seeking an unsolicited grant for a program that fits within our current priorities, you may submit a Letter of Intent (LOI) application at any time. The LOI application will ask for a basic description of your organization and the program or service you propose, a general rationale for the request, and an estimated budget. The Northern California Community Benefit Programs staff will review your online request.If the project appears to be a good match, we will invite your organization to submit a full application and a full application link will be emailed to you by a grant manager. It will include the request for supplemental documentation including the organization’s 501(c)(3) IRS letter, etc.
  1. Full Application. In order to submit a full application, you must receive an email invitation directly from a Northern California Community Benefit Programs staff member. That invitation will include the link to the online application.In addition to completing the online application, you will be asked to complete a supplemental workplan and budget, using the templates below, and to submit a list of required compliance documents.

Letter of Intent (LOI) Applications

LOI Application (IGAM):
To begin your Letter of Intent (LOI), please go to our online application website.

  • To create a new account, click on “I am a new online applicant.” You will be required to enter an e-mail address and create a password.
  • Once you have an account, the system will generate a confirmation e-mail that will include the Account Login URL and your password.
  • Please save this e-mail and bookmark the URL and password for future reference.
  • At this point you will be able to start your online LOI and submit it to our system electronically.

Full Grant Proposal Applications

Full Application (IGAM):
If you received an email from a Community Benefit Programs Department staff member, it will include a link to our online application system (IGAM).

  • If you have already applied online, simply log into your account and proceed with your application.
  • To create a new account, click on “I am a new online applicant” You will be required to enter an e-mail address and create a password.
  • Once you have an account, the system will generate a confirmation e-mail that will include the Account Login URL and your password.
  • Please save this e-mail and bookmark the URL and password for future reference.
  • At this point you will be able to start your online application and submit it electronically

Templates to Complete and Submit:
In addition to your IGAM application, please complete the following templates and include them with your submission. We suggest you download these files, complete them, and then attach them to the online application via the IGAM system.

Supplemental Documents Required:
In addition to the online grant application, we require the following attachments, which must be attached via our IGAM system (it will prompt you at the end of your application):

  • Project Budget
  • Workplan
  • Request on letterhead from the requesting agency and fiscal sponsor, if applicable. Note: Letter should include the legal name of organization, organization’s address, requested amount, project summary and project title
  • Copy of current IRS determination letter indicating tax-exempt status
  • Organization’s most recent Form 990
  • Organization’s most recent audited financial reports
  • List of current Board of Directors and their affiliations
  • Current itemized organization operating budget
  • List of names, titles, and qualifications of key staff and volunteers

General Tips for a Successful Submission

  • For frequently asked questions, you can refer to our online Application FAQ.
  • Access the online LOI application or the full application via the URL e-mailed to you.
  • Create one user account for the organization applying for funding. Once you create your user account you will be taken to the online application. This link provides you with access to our grant guidelines and instructions. Tip: Do not begin entering data into the application yet. Click on “Save & Finish Later.”
  • We suggest that you bookmark the “My Account” page URL, as you will be required to use it to complete and submit your application.
  • In addition, through the “My Account” page you can view a history of open applications and completed submissions as well as email yourself a copy of a draft or completed submission.
  • Prior to submitting your online application, you will be able to review your grant application. Please note that once a grant application has been submitted, it cannot be edited.
